|
MEDICAL INSURANCE
Jagemann Stamping offers PPO and HSA options for medical insurance coverage
for full time employees, with both in and out of plan coverage levels. The plan
also covers prescription drugs and wellness benefits, as well as dental coverage.
Optional vision care is available as well.
SHORT TERM DISABILITY Short term disability
is available at a rate of 100% of base salary for 40 hours. The waiting period is
the first day of an accident or inpatient hospitalization, or the eighth day of
illness. The short term benefit duration is 26 weeks.
LONG TERM DISABILITY Long
term disability is available at a rate of 60% of base salary for 40 hours, with
a maximum benefit of $7,500 per month and a minimum of $100 per month. Coverage
begins after 180 days of continuous disability. Your payment may be reduced by other
sources of income and disability earnings. If disability occurs after age 60, the
disability income will be reduced based upon the maximum period payment schedule
up to age 70.
TERM LIFE / AD & D All full-time employees are offered an amount
equal to 100% of their basic annual earnings, not to exceed $150,000. "Basic Annual
Earnings" is defined as base pay at 40 hours, for 52 weeks, but not including bonus
or overtime payment.
FLEXIBLE SPENDING ACCOUNT You can use the flexible spending
account to save taxes by utilizing pre-tax dollars for allowed insurance premiums,
eligible medical care and equipment, along with dependent care. The plan includes
a Premium Only Plan, Medical Care Reimbursement and Dependent Care Coverage.
WELLNESS
PROGRAMS JSC is committed to employee wellness in support of our most valuable resource,
our people. Our healthy workforce also acts as a deterrent to rising medical costs.
To this end, JSC offers several wellness benefits throughout the year. Some of the
programs offered have included: Health Risk Assessments, onsite flu shots, smoking
cession clinics, and nutrition education.
FITNESS CENTER JSC offers a fitness center
for all employees and their spouses. The center is open during all business hours,
and contains many pieces of equipment designed to meet all your fitness goals All
employees and spouses are encouraged to visit the fitness center regularly, and take advantage
of periodic offers such as fitness trainers, and equipment training. Separate and
private shower and locker room areas are provided.
VACATIONS AND HOLIDAYS JSC offers
a vacation schedule for all full time employees, based on length of employment.
We also offer the following as paid holidays: New Year's Eve, New Year's Day, Memorial
day, Independence Day, Labor Day, Thanksgiving Day, the day after Thanksgiving,
Christmas Eve Day, Christmas Day and one floating holiday that can be assigned at
the company's discretion.
QUARTERLY BONUS In addition to competitive wages, JSC
offers a quarterly bonus plan, which may contribute to employee earnings. The company
quarterly bonus plan is discretionary on the part of the company and is normally
based upon company profitability for the quarter.
RETIREMENT PLAN JSC offers competitive
retirement options, including opportunities for 401(k) and pension plan. Discretionary
contributions can be made by the company on an annual basis. Please see the Human
Resources department for more information on each of these plans.
EMPLOYEE ASSISTANCE
PROGRAM Assistance available for problems related to family, marital, drug/alcohol
abuse, financial problems, emotional stress, depression and work related issues
through a local counseling service. Up to two assessment appointments for you or
your family members are provided without cost to you. Should you decide to accept
a referral, you will be assisted in choosing a provider who specializes in your
problem area, or you can elect to continue with the services of our appointed provider.
All charges for services beyond the two assessment visits will be referred to your
health insurance and would be covered in accord with the provisions of that plan.
All services received will remain confidential between our provider and the individual
receiving services. Assessment and referral sessions take place at our provider
offices. Records of service are the property of our provider and not provided to
JSC.
CONTINUING EDUCATION SUPPORT JSC will assist with the payment of tuition and
other educational expenses, up to $500 per year, additional funds can be made available
with approval of the Education Steering Committee. We also have an education assistance
plan for non-work related study, supporting courses and seminars that are strictly
recreational in nature. There is also a scholarship program for children of JSC
Employees who are attending post-secondary education.
